A severe prophecy of wrath is directed at the king, outlining a heavy, sweeping punishment that comes as a direct consequence of his actions [ביאור שטיינזלץ]. God is striking the king and his entire environment, bringing about two central and distinct disasters [מלבי״ם].
The first disaster targets his nation and his family. A massive blow will strike his people, his children, his wives, and all his property. This devastation will take the form of a brutal attack by the sword, falling heavily upon his household [מלבי״ם].
The second disaster is intensely personal, directed at the king himself. He will suffer from a multitude of different diseases. His suffering will culminate in a severe intestinal illness, leading to a prolonged and humiliating death in which his internal organs will physically emerge from his body. While righteous individuals may sometimes pass away from intestinal ailments, their passing is never marked by such a degrading disgrace [מלבי״ם].
