When a person is overwhelmed by distress, their appeal to God often goes beyond spoken words, combining desperate cries with physical gestures of total dependence. In these moments of deep vulnerability, the individual begs for their plea to be accepted and answered. The cry for help marks a specific moment of intense begging [מצודת ציון, ביאור שטיינזלץ]. However, there is a special significance in asking God to hear the mere physical sound of these pleas. During times of severe hardship, a person often struggles to focus their mind and heart completely. By asking God to listen to the voice of the prayer, the individual hopes the plea will be accepted even if their inner intention falls short of perfection [אלשיך].

This vocal plea is paired with the physical act of lifting and spreading the hands [מצודת דוד, ביאור שטיינזלץ, מאירי]. This movement naturally mimics someone reaching upward to ask for and receive help from another [רד״ק]. Beyond a simple request for aid, raising open hands serves as a declaration of moral cleanliness. The worshiper physically shows that their hands have committed no wrong and that their outward words match their true inner feelings, making their prayer worthy of being heard [אלשיך].

The outstretched hands are directed toward the holy inner sanctuary. The primary approach among commentators is that this refers to the Holy of Holies, the innermost room containing the Ark of the Covenant where God's glory rests [אבן עזרא, מצודת ציון, רד״ק, ביאור שטיינזלץ]. Pointing toward this sacred space creates a symbolic ladder, allowing the prayer to rise directly to the divine throne, much like a worshiper faces the holy ark in a synagogue [רד״ק, מאירי]. While some suggest the prayer might actually be taking place inside this physical sanctuary [רד״ק], an alternative perspective views the holy space not as an earthly location at all. Instead, it serves as a metaphor for the heavens themselves, the ultimate destination toward which the person lifts their hands [רד״ק, מאירי].
