אָ֭וֶן אִם־רָאִ֣יתִי בְלִבִּ֑י לֹ֖א יִשְׁמַ֣ע ׀ אֲדֹנָֽי׃

The inner world of human thought often raises deep questions about accountability. People frequently experience passing negative thoughts, prompting the question of whether the mere intention to do wrong is considered a sin in itself. A wicked thought or intended offense might surface strictly within the mind [ביאור שטיינזלץ]. A person might entertain the idea of a sin, as such thoughts are common to everyone, yet never carry it out into physical action [אלשיך, ביאור שטיינזלץ].

The primary approach among commentators is that God responds to these unacted thoughts with mercy. He does not punish a person for negative thoughts alone. Instead, God acts as though He does not see or hear the offense hidden within the heart. Because a bad thought is not accompanied by a physical act, it is not counted as an actual wrong, and it does not cause a person's prayer to be rejected [רש״י, רד״ק, מצודת דוד, תורה תמימה]. There is an exception to this rule regarding thoughts of idolatry or a denial of faith, for which a person is indeed held accountable and judged [מאירי].

A different perspective shifts the focus from divine mercy to a declaration of personal innocence. According to this view, if there had been any hidden sin in the person's heart, God simply would not have listened to the prayer. The very fact that the prayer was answered serves as proof that the individual's heart was completely pure [אבן עזרא].

Another unique approach suggests that the hidden wrong was actually a feeling of doubt. The uncertainty was whether God truly hears prayers, a doubt that essentially compares Him to deaf idols. The individual confesses that even while this doubt was still resting in their heart, God nevertheless paid attention and listened [מלבי״ם].
