Facing hostility and deep personal suffering, a person often has to choose whether to confront their adversaries or remain quiet. The poet explains the reason behind his silence in the presence of his enemies, or alternatively, the motive behind his desperate plea for God's salvation. The primary approach among commentators is that he deliberately avoids responding harshly to his foes or exposing his illness and weakness to them. He fears that if they witness his downfall, they will celebrate his pain and mock him [רש"י, אלשיך, מצודת דוד]. On the other hand, another perspective views his thoughts as a direct prayer to God. In this view, the poet begs God not to abandon him or withhold His rescue, specifically to prevent his enemies from enjoying his destruction [רד"ק, מלבי"ם].

The poet describes the intense reaction of his enemies to even his smallest moments of weakness. Whenever he merely stumbles, makes a mistake, or simply seems about to fall, his foes immediately react with arrogance. They elevate themselves and speak about him with overwhelming pride [אבן עזרא, מצודת ציון].

This dynamic reveals the poet's deepest fear. If a minor slip of the foot is enough for his enemies to exaggerate the situation, swell with pride, and cruelly claim that he is permanently damaged and will limp forever, the alternative is unthinkable. He dreads what they might do, and how intensely they would celebrate, if he were to collapse completely, suffer affliction throughout his entire body, or if God were to abandon him entirely [רד"ק, אלשיך, מצודת דוד, מלבי"ם, מאירי].
