Physical suffering often mirrors a deeper spiritual crisis. Severe bodily deterioration and infected wounds serve as a painful reflection of spiritual failure, demonstrating how physical distress is a direct result of straying from the path of God and sinking into sin.

The primary approach among commentators is that these afflictions are severe blows where blood pools beneath the skin, forming blisters filled with cloudy fluid and pus. These injuries do not heal. Instead, they undergo a continuous process of dissolving and rotting [רד״ק, ביאור שטיינזלץ, מצודת ציון, מלבי״ם]. As the skin breaks down, the wounds emit a terrible, foul odor [רד״ק, ביאור שטיינזלץ]. The physical symptoms actually unfold in a reverse sequence to how they are initially presented. First, the skin melts and decays. Only afterward, when the trapped pus and blood spill out, does the unbearable stench arise [רד״ק, מצודת דוד, מאירי].

This intense agony comes as a direct punishment for sins committed out of sheer foolishness. A person only commits a sin when a spirit of folly takes hold of them, blinding their judgment and distancing them from God [רד״ק, מצודת דוד, מצודת ציון]. While this explains the general nature of the spiritual failure, another perspective determines the exact type of sin based on how quickly the wounds develop. Because the injuries dissolve and rot away immediately rather than lingering on the body, this folly refers specifically to unintentional sins. Had the transgressions been committed intentionally, the wounds would have persisted, tormenting the body for a much longer time [אלשיך].
